Chatham County is one of the 10 North Carolina counties participating in Thriving Hearts. From now through 2030, Thriving Hearts will be working with the Chatham County Health Department to improve maternal health outcomes in the county. Our collaboration builds on current maternity practices and ongoing efforts to improve the quality of care for community members during the pregnancy and postpartum period. Chatham County is considered a small size county based on their number of births each year. Chatham has 1 Perinatal Community Health Worker (PCHW) connecting patients and birthing persons to community resources.
